Surgical sperm retrieval at Gynatal Clinic (Timișoara) encompasses minimally invasive urological techniques (TESE, TESA, PESA) designed to obtain sperm directly from the testes or epididymis. Led by Dr. Vlad Tucicovschi, these procedures allow men with azoospermia to achieve biological parenthood through subsequent ICSI. Gynatal represents the regional "Scientific Gold Standard" in andrological care, combining surgical expertise with advanced Clean-Room embryology. 

When is Surgical Sperm Retrieval Recommended

This intervention is indicated when a standard Spermogram repeatedly confirms the absence of sperm (Azoospermia). At Gynatal, we distinguish between two primary types to determine the appropriate surgical path: 

  • Obstructive Azoospermia (Blocked Excretion): Sperm are produced normally but cannot be ejaculated due to a blockage (e.g., post-vasectomy, infections, or congenital absence of the vas deferens). In these cases, PESA or TESA techniques are highly effective. 

  • Non-Obstructive Azoospermiatesticular failure: The testes produce extremely low levels of sperm. These cases require TESE or advanced MicroTESE to identify microscopic "pockets" of sperm production. 

Advanced Retrieval Methods at Gynatal 

Our andrology department, led by Dr. Vlad Tucicovschi, applies these procedures with a focus on patient comfort and cellular integrity: 

  1. PESA (Percutaneous Epididymal Sperm Aspiration): Sperm is collected from the epididymis using a fine needle under local anesthesia. This is a rapid method for obstructive conditions. 

  1. TESA (Testicular Sperm Aspiration): Aspiration performed directly from the testicular tissue. 

  1. TESE (Testicular Sperm Extraction): A small biopsy of testicular tissue. The fragments are immediately analyzed in our Clean-Room laboratory by the team led by Lect. Univ. Dr. Delia Huțanu to confirm the presence of viable sperm. 

  1. MicroTESE: Performed under an operating microscope, allowing for the targeted collection of the most promising seminiferous tubules while minimizing trauma to the testis. 

 

 

Synergy with ICSI Laboratory Technology

Sperm obtained surgically often have limited motility. Therefore, at Gynatal, they are utilized exclusively in conjunction with ICSI (Intracytoplasmic Sperm Injection), where a single selected sperm is injected directly into the oocyte. To further enhance success rates, we often combine this with IMSI (6000x magnification) to ensure only morphologically superior cells are used.
